Documentation of disability is often necessary in order to help with the determination of accommodations. In situations when the disability is apparent and there is a clear link between the functional limitation of the condition and the request for accommodation, SDAC may waive the need for documentation (i.e. a student broke their arm and requested assistance with notetaking in class). In other situations, when it is not apparent, SDAC may require documentation as part of the eligibility process. Documentation can come in many forms, such as self-report, history of accommodations, or documentation from an appropriate treating provider. Specific guidelines are available below.
If you do not currently have documentation, then SDAC has a Verification of Disability form which may be submitted to a treating provider and returned to the SDAC.
