COVID-19 Vaccine Requirement

On January 7, 2022, in response to a significant spike in COVID-19 cases as a result of the omicron variant, and based on the advice of our public health experts, University leaders moved up the deadline for UVA Academic Division students, faculty, and staff, and students in the School of Medicine and the School of Nursing, to upload proof of a COVID-19 booster shot from February 1, 2022 to January 14, 2022. Those who are not eligible to receive their booster by January 14, 2022, must submit proof of their booster no later than 30 days following their first day of eligibility.

  • To upload proof of your vaccination and booster, take a photo of your vaccination record (CDC vaccine card, etc.), login to the HealthyHoos patient portal, click "Upload," and select "COVID-19 Vaccine Immunization Information." Note that uploads must include the name of the vaccine manufacturer and, when possible, should be in English. To verify that your upload was successful, click on the link titled “Documents already on file” (you may need to refresh the page to see your latest upload).

If you previously received a medical or religious exemption from the University’s vaccine requirement, you do not need to take any further action, other than to continue complying with testing requirements and other public health policies. Those who have never requested a religious or medical exemption or whose circumstances have changed since their first exemption request can download one of the exemption forms below, complete it, and upload it to the HealthyHoos patient portal for review. Please note that a recent case of COVID-19 does not qualify as grounds for a medical exemption.
