Before returning to UVA after a medical withdrawal for psychological or psychiatric reasons, you must complete the Medical Reenrollment Review with Counseling and Psychological Services.
Our goal is to facilitate a successful return to school. You are encouraged to review the Withdrawal and Reenrollment Checklist and speak with your Dean regarding deadlines for reenrollment and academic eligibility before submitting these forms. Ultimately, your Dean will make the final decision regarding your reenrollment.
Instructions for Submission:
Upload the following documents to the Healthy Hoos patient portal. You may use Adobe Fill and Sign to complete the Student form. If you are unable to submit via Healthy Hoos, you may submit via email to firstname.lastname@example.org using password protection. Please call 434-243-5150 to provide your chosen password to our front desk.

This form to be completed by the provider who treated you during your absence from the University. If the documentation from a treating professional is not adequate or available, you have not received treatment while away, or due to other circumstances surrounding your medical withdrawal, you may be asked to complete an assessment with a CAPS clinician as part of this review.

Please allow up to 10 business days after both forms have been submitted for us to process your request, and monitor Healthy Hoos messages and your UVA email in case we request additional information. You, your Association Dean, and the Office of the Dean of Students will be notified via email once your Medical Reenrollment Review is complete.
