Students who schedule an appointment within the Department of Student Health and Wellness (SHW) will receive information about an advance directive by secure message. SHW is required to inform patients of their right to implement an advance directive.
- An advance directive is a legal document that explains how you want medical decisions about you to be made if you cannot make the decisions yourself. It only goes into effect if you are incapacitated and unable to speak for yourself. An advance directive lets your health care team and loved ones know what kind of health care you want, or who you want to make decisions for you when you can't.

If you would like an advance directive as part of your medical record, please complete the form and upload it into your HealthyHoos patient portal.
- Go to the upload tab.
- Locate the section for ADVANCE DIRECTIVE/DDNR UPLOAD ONLY
- Upload copies of the entire document, front and back.
- Ensure your document is signed by two witnesses.
- After your advance directive is reviewed, it will be made available in your patient chart.
- At your next visit, please inform your health care provider of your advance directive.
