Pre-Entrance Health Requirements

In compliance with state law and university policy, all full-time students and all health professions students are required to complete pre-entrance health requirements as outlined below.

Requirements are due before the start of classes (August 1st fall entry, Jan 31st spring entry). Records submitted after the deadline will incur a late charge with the potential of a registration hold.

Charges and Holds for Late Submissions

Failure to complete and submit pre-entrance health requirements by August 1st for fall entry or January 31st for spring entry will result in a late charge of $100. 

Continued failure to satisfy all pre-entrance health requirements will result in a registration hold for the student’s second semester that will prohibit registering for classes. 

 

Collecting Prior Immunization Information

  • Students who received vaccines in the state of VA only: As a courtesy to you, Student Health and Wellness will query the Virginia Immunization Information System (VIIS) to pull information regarding your vaccination history. If you have any missing requirements, or we cannot access your information, we will send you a message with details.
  • Students who received vaccines in the United States, but outside of VA (except AK, AR, HI, NH, ME, VT, and DC):  You can request that your immunization records from other states be sent to Student Health and Wellness. You must initiate this process; our staff cannot do it on your behalf.  To initiate this request, log into the HealthyHoos Portal and provide address information for states where you have lived and received immunizations so immunization information can be retrieved. If you have any missing requirements, or we cannot access your information, we will send you a message with details. Please Note:  Pennsylvania, New York, Utah and Nevada Immunization Registry Systems are temporarily unavailable with no anticipated availability date.  It is recommended that students who have records from these states complete the Pre-Entrance Health Form or upload immunization documents.
  • Students who have received vaccines outside of the United States or in states of AK, AR, HI, NH, ME, VT, and DC:  Please complete the Pre-Entrance Health Form (see Forms and Documentation section) and upload it to the HealthyHoos Portal.

Step by Step Instructions

Requesting Immunization Records Through HealthyHoos

This is for students who have received immunizations in the United States, but outside of VA (except AK, AR, HI, NH, ME, VT, and DC)

1. To request immunization records, log-in to the HealthyHoos Portal.

2. Click “Immunizations” on the left side of the screen.

3. Click the blue “Request My Out of State Immunization Registry Records” button  

4. Two options will appear: 

     a. If you lived in one state and received immunizations in other states while living there, follow step 5.

     b. If you lived in more than one state and possibly received immunizations in each one, follow step 6.

 

5. Fill out the demographic information and enter the address to the best of your abilities.  The most important piece of information is the state. 

    a. Answer “Yes” to the “Did you receive vaccines in any other state while living at this address?”.

    b. Select the states from the list which you received immunizations while living at the address listed.

    c. Click “Submit”.

6. Fill out the demographic information and enter the address to the best of your abilities.  The most important piece of information is the state. 

    a. Answer “No” to the “Did you receive vaccines in any other state while living at this address?”. 

    b. Click “Submit”.


7. A Consent screen will appear and after you have read it,  click “Accept”.

8. If all the states you received immunizations are listed, click the green “Finish and Submit” button.

    a. If you need to add additional states, click the blue “Add another state” button and follow steps 5 and 7 or 6 and 7 again until all the states you received immunizations are listed, then click the green “Finish and Submit” button.
 

9. A pop up will appear thanking you for your immunization registry,  click “OK”.

10.  You will now see the immunization request is under “Submitted Requests” with a Status. 

11.  Once your request has been processed, you will receive an email from [email protected] indicating your immunization request was updated. 

12.  You should receive communication from the Medical Records Team after your immunization request has been processed, however, if you would like to check your immunization status, log in to the HealthyHoos Portal and click on “Immunizations”.  Click the green “Check your immunization status” button.

13. Click the green “Print” button.  

14. Another screen will open up with an immunization record. Immunizations that still need to be completed to fulfill the pre-entrance health requirements will be at the bottom of the screen under “Missing University Requirements”.

Uploading a Pre-Entrance Health Form or Other Immunization Documentation

1. To upload a form or immunization documentation, log-in to the HealthyHoos Portal.

2. Click “Uploads” on the left side of the screen.

3. Before uploading, please read the directions at the top of the page carefully.  Common errors that we see are:

     a. Files that are OVER 4 MB which the system will not allow.  Please make sure it is 4 MB or less.  Some students find taking a picture of each page and uploading them separately is a good option if you cannot get the file size small enough. 

     b. Long and complex file names which the system will not allow.  Please keep files names short, such as “pehf” or “immunizations”.  The system will not allow more than 20 characters and no symbols in file names.
 

4. After reading the directions, scroll down the page to the “Pre-Entrance Health Form/Immunizations” section and click “Select File”.

5. A screen will open up to find and select the file you wish to upload.  Select the file and click “Open”.

6. Once you have uploaded the file, you will see it where the “Select File” button was.  You can click “Change” to change the file you selected, or go to Step 7 if this is the file you want to upload.

7. Scroll down to the bottom of the screen and click “Upload” on the bottom right of the screen.

8. Once the file is uploaded successfully, you should see the document upload information at the bottom of the screen under “Documents already on file”.  You can repeat this process to upload multiple documents, if necessary.

Completing the TB Screening Questionnaire on HealthyHoos

1. To complete the TB Screening Questionnaire, log-in to the HealthyHoos Portal.

2. Click “Forms” on the left hand side of the screen.

3. At the top of the screen, click “HealthyHoos TB Screening Questionnaire”.

4. Complete the questionnaire by answering each question and typing your name in the signature box and then hit “Submit”.

5. Once you have completed the questionnaire you will receive a message thanking you for completing the TB Screening Questionnaire.  Click “OK”.

6. You will then be taken to the “Forms” Home Screen where you will see information about your questionnaire completion.

7. The Medical Records Team will process the information on the questionnaire and will send a secure message if further evaluation is required.

 

Required Vaccines

  • Tetanus (Td or Tdap): 1 dose within the past 10 years
  • Measles, Mumps, Rubella*: 2 MMR doses- 1st dose on or after 1st birthday and doses at least 28 days apart
  • Polio*: completed childhood series or 1 dose of IPV on or after 18th birthday
  • Hepatitis B*: completed series
  • Meningococcal ACWY (students <22 yrs or age): dose on or after 16th birthday

Recommended Vaccines

*Titer demonstrating immunity is an acceptable alternative to vaccination. Lab report must be uploaded to the HealthyHoos Portal.

Required Tuberculosis Screening

  • All full-time students and all health professions students must complete the online TB Screening form on the HealthyHoos Portal. Click on “Forms” and select “HealthyHoos TB Screening Questionnaire”.
  • Tuberculosis Assessment Form (page 3-4 of the Pre-Entrance Health form): Students must complete the TB Screening Questionnaire in HealthyHoos Portal prior to filling out this form.  The Tuberculosis Assessment form is Required for all Health Professions Students.  Additional students may be required to complete this form based on Student Health and Wellness guidance after review of the TB Screening Questionnaire. 

Additional Requirements for Health Professions Students

School of Medicine (MD, BIMS, PHS), School of Nursing (Undergraduate, Graduate), and Biomedical Engineering (Graduate) Students

  • Tdap: one adult dose (on or after 10th birthday)
  • Hepatitis B Quantitative Antibody: confirmation of immunity
  • Varicella*: 2 doses
  • Flu (annually).  Documentation of flu vaccine must be submitted by following your school’s directions and deadline. Typically, the annual flu vaccine becomes available in early fall and must be obtained by the end of November (deadline subject to change).

Consent for the Treatment of Minors Form

It is recommended that students under 18 years of age submit a Consent for the Treatment of Minors Form.
