Pre-Entrance Health Requirements

Prior to your arrival at UVA, Student Health and Wellness needs information about your health status and health insurance coverage. 

  • To fulfill the health insurance requirement, students will receive information regarding how to submit when the portal opens on July 15, 2024.

  • Students should create their Netbadge ID and register their official UVA email account (Darden, Law, Mcintire, and UVA Health - please set up and use your @virginia.edu email address, not your school-specific email.) Students must activate both their virginia.edu email address and their Netbadge ID to access HealthyHoos.

Key Deadlines

If you are a NEW student (undergraduate, transfer, graduate):

1. Pre-Entrance Health Form (PEHF): Please complete the PEHF accurately and upload it to the HealthyHoos patient portal by August 1 (fall start) and January 31st (spring start.) If your form is not received by the due date, you will be subject to a late fee of $100. 

  • When possible, forms (included the PEHF) and immunization records uploaded or submitted to SHW should be filled out in English to expedite the review process. If English is not possible, our Medical records team may reach out to you to provide additional information as needed.
  • If you wish to appeal the late fee placed on your SIS account or a hold on your registration, please use this appeal form.

2. Health Insurance Requirement: All UVA students are required to have health insurance coverage. Students can satisfy this requirement by waiving UVA Aetna Student Health Plan coverage and uploading proof of current coverage or by enrolling in the UVA  Student Health Plan (available from July 15 to August 30). Students must complete the health insurance verification process by August 30 (fall start) and January 31st (spring start.)

If you are a RETURNING student (undergraduate, transfer, graduate):

1. Health Insurance Requirement: All UVA students are required to have health insurance coverage. Students can satisfy this requirement by waiving UVA Student Health Plan coverage and uploading proof of current coverage or by enrolling in the UVA Student Health Plan (available from July 15 to August 30). Students must complete the health insurance verification process by August 30(fall start) and January 31st (spring start.)

Please note, only MEDICAL or NURSING students should use the following PEHF. If you are enrolled in any other academic program or school at UVA, please use the "Undergraduate, Graduate, Transfer Students" PEHF above.
